The Simplera app uploads sensor history to the CareLink Personal website (carelink.minimed.com), where the wearer can view reports on a computer. (Medtronic user guide, p. 44)
Simplera (standalone) sends readings to the Simplera app on a phone; Simplera Sync pairs with the MiniMed 780G pump, and the MiniMed Mobile app is a secondary display for the pump.
